Want to become a member of Friendship Place, Inc. in order to participate in activities and all of the benefits of membership? It's FREE and takes about 10 minutes.

1)   A person needs to have a primary diagnosis of a Chronic Mental Illness.

2)   An Admission form needs to be filled out and documentation needs to be provided, showing that the indidividual is/has receiving(ed) services through a county mental health division or is/has been actively under the care of a psychiatrist, psychologist, therapist, or caseworker for their primary diagnosis of a chronic mental illness.

3)   Membership card will be issued upon completion of admission policy/forms.

In order to complete the admission for membership process, prospective members will need to present the above-mentioned forms (completed) and meet with a staff member. Upon meeting with staff and completing all required paperwork (takes less than 10 minutes), you will be issued a free membership card.
